A 1.5-inch segment consisting of roughly 200 hair strands (similar to the diameter of a #2 pencil) nearest to the scalp yields 100mg of hair, deemed the perfect sample for screening and confirmation. For EtG, additional tests, and tests exceeding 10 panels, 150mg of the sample is advisable. We suggest using a jeweler's scale for weighing the specimen. If scalp hair is not present, a comparable amount of body hair can be collected. When discussing head hair, we mean scalp hair exclusively. Body hair encompasses all other types of hair (e.g., facial, axillary).

Process Overview

The laboratory process for a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first step of integrating a sample into the laboratory's system. This step ensures the sample is properly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and completes any remaining data entry absent from an electronic chain of custody system.

Screening is the initial, swift evaluation for drugs of abuse. Although Screening offers a cost-effective means to exclude drug use in most samples, a confirmed positive result is required for court admission. Any presumptively positive samples during Screening mandate a secondary confirmation.

When a sample shows a presumptive positive result during Screening, additional hair is extracted from the initial sample for the Extraction stage. Here, drugs are separated from hair at significantly lower concentrations compared to other methodologies (e.g., urine or oral fluid), presenting hair drug screening as the most intricate methodology to execute.

Positive results from screening undergo Confirmation using GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all presumptive positive samples are washed as necessary. The entire process, from Accessioning to Confirmation, is audited under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standard accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
