What it is, How it works

To collect a sample, a 1.5-inch segment of about 200 hairs, roughly equivalent to a #2 pencil, closest to the scalp will produce 100mg of hair, which is the perfect amount for screening and confirmation. For EtG enhancements, and/or tests over 10 panels, it is advisable to collect 150mg of the specimen. Measuring the specimen on a jeweler’s scale is advised. If scalp hair is not available, an equivalent amount of body hair can be gathered. Reference to head hair pertains solely to scalp hair, while body hair includes all other hair (facial, axillary, etc.).

Process Overview

The primary steps in processing a drug test result in the lab are comprised of Accessioning, Screening, Extraction, and Confirmation.

Accessioning deals with the initial input of a sample into a lab’s database. This process confirms the sample was appropriately sealed and dispatched, assigns a unique LAN (Laboratory Accessioning Number), and completes any data entry not managed by an electronic chain of custody system.

Screening involves a preliminary assessment for drug abuse indicators. While it’s a cost-efficient method to eliminate drug usage in most cases, a positive screen necessitates confirmation to be legally permissible. Any samples flagged as possibly positive in Screening require further confirmation.

In instances where a sample is potentially positive in the Screening phase, additional hair is extracted from the initial sample for Extraction. In this segment, drugs are extracted at significantly lower concentrations than with other methods (e.g., urine or oral fluid), rendering hair drug testing a challenging methodology to carry out.

Confirmation for any positive screening result is executed through GC/MS, GC/MS/MS, or LC/MS/MS. All provisional positive samples undergo washing before confirmation if required. The entire lab procedure from Accessioning to Confirmation undergoes review under the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
