A 1.5-inch segment consisting of roughly 200 hairs (comparable to the diameter of a #2 pencil) taken closest to the scalp will yield about 100mg, the optimal amount for initial and confirmatory testing. For extended tests like EtG, augmentations, and/or those exceeding 10 panels, 150mg is advised. Using a jeweler’s scale to weigh the sample is suggested. Should scalp hair be unavailable, an equivalent amount of body hair can be substituted. When mentioning head hair, we specifically mean scalp hair. Body hair includes all other hair types (such as facial, underarm, etc.).

Process Overview

The laboratory processing of a test result for drugs involves four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes entering a sample into the lab's system. The process ensures the sample's proper sealing and dispatch, generates a random LAN (Laboratory Accessioning Number), and requires any remaining data entry not covered by an electronic chain of custody.

Screening provides a preliminary check for the presence of drugs. While cost-effective for dismissing drug use in most samples, a positive screen requires confirmation to be valid legally. Samples with positive results in Screening need further verification.

In cases of a positive Screening result, more hair from the initial sample is utilized for Extraction. Here, drugs are extracted at considerably lower concentrations than in alternative methods (such as urine or saliva), explaining the complexity of hair drug analysis.

Positive screenings are confirmed via GC/MS, GC/MS/MS, or LC/MS/MS. Prior to Confirmation, all samples with preliminary positives are cleaned as necessary. This entire laboratory sequence, from Accessioning to Confirmation, undergoes scrutiny under both the CAP (College of American Pathologists) Hair standards and the ISO / IEC 17025 accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
