What it is, How it works

Hair strands offer a prolonged timeline of alcohol and drug usage by encapsulating biomarkers within the structure of the hair as it grows. When collected near the scalp, hair can reveal drug or alcohol use for approximately three months. Hair collection is straightforward, challenging to tamper with, and convenient to transport.

A sample measuring 1.5 inches consisting of roughly 200 hair strands (or the diameter of a #2 pencil) taken from the scalp provides about 100mg of hair, which is optimal for screening and confirmation. For EtG, additional tests, and/or testing exceeding 10 panels, a 150mg specimen is advisable. We suggest weighing the sample using a jeweler's scale. In the absence of scalp hair, an equivalent quantity of body hair may be gathered. References to head hair solely indicate scalp hair. Body hair denotes all other hair types (such as facial, axillary, etc.).

Process Overview

The key stages in the laboratory processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the preliminary entry of a sample into a lab's system. This step verifies that the sample was correctly sealed and shipped, assigns a unique LAN (Laboratory Accessioning Number), and completes any necessary data input not covered by an electronic chain of custody system.

Screening represents an initial rudimentary assessment for illicit drug use. While Screening is a cost-effective manner of excluding drug use for the majority of samples, a positive result necessitates confirmation for legal admissibility. Samples that test positive during Screening require further verification.

In cases where a sample is positive at the Screening stage, additional hair is taken from the initial sample and readied for Extraction. During this phase, drugs are extracted from hair in far lower concentrations than with other methodologies (e.g., urine or oral fluid), which accounts for the complexity of hair drug testing.

Positive Screening results undergo Confirmation using methods like GC/MS, GC/MS/MS, or LC/MS/MS. All samples deemed presumptively positive are cleansed before Confirmation as necessary. The entire lab process, from Accessioning through Confirmation, is scrutinized under the CAP (College of American Pathologists) Hair standard and is accredited to the ISO / IEC 17025 benchmarks.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
