A sample measuring 1.5 inches that consists of around 200 hair strands (similar in size to a #2 pencil) from near the scalp yields 100mg, the optimal quantity for both screening and confirmation. For tests involving EtG, additional tests, and/or more than 10 panels, 150mg of the hair is preferred. We suggest using a jeweler’s scale for weighing the sample. If scalp hair cannot be collected, an equivalent quantity of body hair may be used. In our context, head hair refers solely to scalp hair, whereas body hair pertains to other types of hair (face, underarm, etc.).

Process Overview

The laboratory procedure for a drug test result includes four key phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the first stage of entering a sample into a lab’s system. This process involves confirming the sample was adequately sealed and sent, assigning a random LAN (Laboratory Accessioning Number), and inputting any additional information not provided by an electronic custody chain.

Screening provides an initial quick evaluation for drugs. While Screening is a cost-efficient method for ruling out drug consumption in most samples, a positive screening must be confirmed to hold up in court. Samples that are preliminarily positive in Screening must undergo a secondary confirmation.

If a sample shows a preliminary positive in the Screening phase, more hair is extracted from the sample and prepped for Extraction. During Extraction, drugs are pulled from hair at much lower concentrations than other methods (e.g., urine or oral fluid), underscoring why hair drug testing is the most complex method to execute.

Confirmation of any positive screening result is carried out through GC/MS, GC/MS/MS, or LC/MS/MS. Samples with presumptive positive results are washed before necessary confirmations. The entire laboratory sequence from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair guidelines and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
