For tests including EtG, add-ons, and those exceeding 10 panels, a sample of 150mg is advised. Utilizing a jeweler’s scale for weighing the sample is recommended. If scalp hair isn't accessible, a similar quantity of body hair can be used. It's important to note that head hair references only scalp hair, while body hair encompasses all other types (such as facial, axillary, etc.).

Process Overview

The primary steps in the laboratory processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial integration of a sample into a lab’s system. This involves ensuring the sample was properly sealed and sent, assigning a random LAN (Laboratory Accessioning Number), and completing any data entry not included by an electronic chain of custody system.

Screening serves as an initial quick evaluation for drugs of abuse. Though Screening is a cost-efficient method to exclude drug usage in most samples, a positive screen must be confirmed to stand up in court. Any samples that test presumptively positive in Screening require a follow-up confirmation.

Should a sample test presumptively positive in the Screening phase, additional hair is taken from the initial collection and readied for Extraction. In this phase, drugs are withdrawn from hair at much lower concentrations than in other techniques (e.g., urine or oral fluid), making hair drug screening one of the most complex methods to conduct.

Confirmation of any positive screening result is achieved using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation if necessary. The complete laboratory process from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair designation along with the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
