A 1.5-inch segment containing roughly 200 hair strands (comparable to the size of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the optimal sample size for screening and validation. For EtG, additional tests, and/or panels exceeding 10, a specimen weight of 150mg is advised. It is recommended to weigh the sample using a jeweler’s scale. If scalp hair can't be obtained, a similar amount of body hair may be used. References to head hair pertain only to scalp hair. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result consists of four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial handling of a sample entering a lab’s system. This step verifies that the sample was properly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and finishes any additional data entry absent from an electronic chain of custody system.

Screening conducts an initial rapid assessment of drugs of abuse. While Screening is a cost-saving approach for ruling out drug usage in most samples, any positive screening necessitates confirmation to be valid in court. Samples showing positive results in Screening require further confirmation.

If a sample shows a positive indication in the Screening phase, more hair from the original sample is extracted and prepared for Extraction. In this phase, substances are extracted at much lower concentrations than with other methods (e.g., urine or oral fluid), making hair drug screening a highly challenging methodology to execute.

Any positive screening outcomes are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are cleaned as necessary before confirmation. The complete laboratory workflow from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 benchmarks.
