A 1.5-inch section of roughly 200 hair strands (approximately the width of a #2 pencil) nearest to the scalp yields 100mg of hair, which is perfect for evaluation and confirmation. For EtG, additional tests, and panels exceeding 10, a 150mg sample is advisable. We suggest utilizing a jeweler’s scale for measuring the sample. If scalp hair isn't available, an equivalent quantity of body hair may be gathered. References to head hair pertain specifically to scalp hair. Body hair includes all other varieties (facial, axillary, etc.).

Process Overview

The primary phases in processing a lab test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning deals with the initial sample processing into a laboratory’s system, which comprises validating the sample’s proper sealing and shipping, assigning a LAN (Laboratory Accessioning Number) randomly, and completing any extra data entry not available through an electronic chain of custody mechanism.

Screening encompasses a preliminary assessment for possible drug abuse. While Screening is economically efficient to exclude drug use in most samples, a positive finding must be confirmed for legal admissibility. Samples preliminarily positive in Screening require additional confirmation.

Should a sample initially test positive during Screening, additional hair is retrieved from the initial sample for Extraction. In this phase, drugs are isolated from hair at significantly lower concentrations than seen in other methodologies (e.g., urine or oral fluids), which makes hair drug testing particularly challenging.

Confirmation for any positive screening finding is performed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All preliminary positive samples undergo washing before confirmation as necessary. The entire lab procedure from Accessioning to Confirmation is scrutinized by standards of both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
