For EtG, extra tests, or those exceeding 10 panels, a sample of 150mg is advisable. We suggest weighing the hair sample using a jeweler’s scale. In cases where scalp hair is not available, a similar quantity of body hair can be used. In this context, head hair refers exclusively to scalp hair, while body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The primary phases in processing a drug test result at the lab include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial integration of a sample into a lab's framework. This step includes ensuring the sample was sealed and dispatched correctly, allocating a random LAN (Laboratory Accessioning Number), and completing any remaining data input not handled by an electronic chain of custody system.

Screening acts as an initial, swift assessment for substances of abuse. While it provides a cost-efficient method to rule out drug use in most samples, any positive findings require confirmation for legal admissibility. Samples showing presumptive positives during Screening necessitate a secondary confirmation.

When a sample tests presumptively positive during Screening, additional hair is extracted from the original sample for the Extraction phase. In this phase, the concentration of drugs is extracted from hair, which is lower than methods like urine or oral fluid, making hair drug screening the most challenging method to execute.

Confirmation of any positive Screened result is achieved using GC/MS, GC/MS/MS, or LC/MS/MS. Every presumptive positive sample undergoes a cleansing before confirmation as needed. The entire lab procedure from Accessioning to Confirmation is scrutinized under the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
