For EtG, add-ons, and/or more than 10 panels, 150mg of the sample is advised. It's best to weigh the sample with a jeweler’s scale. If scalp hair isn’t available, an equivalent amount of body hair can be used. Head hair refers strictly to scalp hair. Body hair encompasses all other types such as facial or axillary hair.

Process Overview

The laboratory processing of a drug test involves four main stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary processing of a sample into a lab's system. This includes ensuring the sample was sealed and transported properly, giving it a random LAN (Laboratory Accessioning Number), and completing any further data entry not covered by an electronic chain of custody system.

Screening is a quick preliminary check for drugs of abuse. Although Screening is an economical way to exclude drug use in most samples, a positive screen requires confirmation to be court-admissible. Any samples showing presumptive positives in Screening require an additional confirmation.

When a sample is presumptive positive in the Screening phase, more hair from the original sample is taken and prepped for Extraction. Drugs are extracted from hair at a significantly lower concentration compared to other methods (e.g., urine or oral fluid), highlighting why hair drug screening is the most challenging methodology.

Confirmation of any positive screening is achieved with GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed if needed before confirmation. The whole lab process from Accessioning to Confirmation is audited under both the CAP (College of American Pathologists) Hair criteria and the accreditation to ISO / IEC 17025 standards.
