What it is, How it works

Hair testing has emerged as an effective method for detecting drug and alcohol consumption. Hair captures a long-term record of alcohol and drug use by preserving biomarkers within the hair strand's fibers. When obtained near the scalp, hair offers up to roughly a 90-day detection period for alcohol and substances. Collecting hair is straightforward, relatively hard to tamper with, and convenient to transport.

A sample of 1.5 inches with around 200 hair strands (similar to the size of a #2 pencil) close to the scalp will yield 100mg of hair, which is the preferred quantity for screening and confirmation. For EtG, additional tests, and/or screens beyond 10 panels, a 150mg specimen is advised. We suggest measuring the specimen using a jeweler’s scale. If scalp hair is not an option, a comparable quantity of body hair can be gathered. When mentioning head hair, we specify scalp hair exclusively. Body hair includes all other hair forms (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial integration of a sample into a laboratory’s workflow. This process ensures the sample's proper sealing and shipping, assigns a random LAN (Laboratory Accessioning Number), and completes any missing data entry if an electronic chain of custody method isn't used.

Screening serves as an initial rapid assessment for drug abuse. Though Screening effectively rules out drug use in most samples at a lower cost, a positive outcome requires confirmation for legal validity. Samples that test presumptively positive in Screening must undergo a secondary confirmation process.

If Screening suggests a positive result, additional hair is drawn from the initial sample for Extraction. During this phase, drugs are extracted from hair in much lower concentrations than other methods (e.g., urine or oral fluid), rendering hair drug testing a complex methodology.

Confirmation of any positive results from Screening is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all samples that tested presumptively positive are washed as necessary. The comprehensive laboratory process from Accessioning to Confirmation is validated under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
