Hair collection is straightforward, moderately resistant to tampering, and is easily transportable.

A 1.5-inch portion consisting of around 200 strands (resembling the size of a #2 pencil) nearest to the scalp yields 100mg, the ideal quantity for analysis and verification. For EtG, additional tests, and those exceeding 10 panels, 150mg of the sample is advisable. Utilizing a jeweler’s scale to weigh the sample is suggested. If no scalp hair is present, an equivalent quantity of body hair can be used. When discussing head hair, we specifically mean scalp hair. Body hair signifies all other hair types (facial, axillary, etc.).

Process Overview

The primary four stages in laboratory analysis of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary process of integrating a sample into a lab’s protocol. This involves verifying the sample's seal and proper shipping, assigning a random LAN (Laboratory Accessioning Number), and completing any additional information not captured by an electronic custody chain system.

Screening involves a preliminary rapid check for drug abuse. While Screening provides an economical approach to potentially exclude drug involvement in most cases, a positive screening necessitates validation to be recognized in legal proceedings. Samples deemed potentially positive in Screening require additional confirmation.

If Screening suggests potential positivity, additional hair is taken from the original sample and prepped for Extraction. Here, drugs are isolated from hair at significantly diluted concentrations compared to other methods (e.g., urine or oral fluid), which is why hair drug testing presents the greatest difficulty.

Verification of any positive screening is executed through GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, presumptive positive samples undergo necessary cleaning. The entire laboratory process from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair guidelines and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
