What it is, How it works

An ideal sample for analysis and confirmation consists of a 1.5-inch section of roughly 200 hair strands (comparable to a #2 pencil diameter) nearest to the scalp, equating to 100mg of hair. For EtG, supplementary analyses, or tests exceeding 10 panels, a larger 150mg sample is advised. It's recommended to measure the sample’s weight with a jeweler’s scale. If scalp hair isn’t available, an equivalent amount of body hair may be collected. By head hair, we mean scalp hair exclusively. Body hair pertains to all other hair forms (e.g., facial, axillary).

Overview of Process

The laboratory processing of a drug test result involves four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial integration of a sample into a laboratory’s system. This step involves confirming the sample’s proper sealing and shipment, assigning a unique LAN (Laboratory Accessioning Number), and completing any remaining data entry absent from an electronic chain of custody platform.

Screening denotes a preliminary inspection for substances of abuse. While Screening is an economical method to exclude drug usage for most samples, a positive screen necessitates confirmation for legal admissibility. Samples that yield presumptive positive results during Screening must undergo further confirmation.

When a sample presents as presumptive positive in Screening, additional hair is drawn from the original specimen and prepped for Extraction. In this stage, substances are isolated from hair at considerably lower concentrations compared to alternative methods (e.g., urine or oral fluid), which is why hair drug testing is the most challenging methodology to execute.

Confirmation of any screen positive is carried out using GC/MS, GC/MS/MS, or LC/MS/MS technologies. Before confirmation, presumptive positive specimens are washed as necessary. The entire lab process, from Accessioning to Confirmation, meets review under the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
