What it is, How it works

Hair analysis has emerged as an influential method for uncovering alcohol and drug consumption. The strands of hair capture biomarkers, presenting a prolonged history of substance use within the fibrous structure. With strands gathered near the scalp, a detection period of up to about three months can be provided, specifically for substances like alcohol and other drugs. Collecting hair is straightforward, challenging to tamper with, and convenient for shipping.

An optimal 1.5-inch sample, comprising roughly 200 hairs (similar in size to a #2 pencil) positioned nearest to the scalp will equate to 100mg, the perfect amount for both screening and confirmation. For assessments like EtG, additional panels, or extensive evaluations exceeding 10 panels, a 150mg sample is advisable. We recommend utilizing a jeweler’s scale for precise measurements. If scalp hair isn't available, an equivalent mass of body hair can be obtained. In the context of head hair, we solely mean scalp hair. Body hair includes all other variations (e.g., facial, axillary).

Process Overview

The principal steps in processing drug test results in a lab include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial incorporation of a specimen into the lab's processing system. This step includes confirming the proper sealing and shipping of the sample, generating a random LAN (Laboratory Accessioning Number), and executing any additional data input absent from an electronic chain of custody method.

Screening entails an initial, swift assessment for substances likely to be abused. Although it's an economical way to eliminate drug presence in the majority of samples, a positive outcome in screening necessitates confirmation for court admissibility. Presumptively positive samples in Screening must undergo an additional confirmation procedure.

Should a specimen test presumptively positive during Screening, more hair is drawn from the initial collection for the Extraction phase. In Extraction, substances are retrieved from hair at significantly lower concentrations relative to other methods (e.g., urine or oral fluid), rendering hair drug analysis as the most complex process to undertake.

Any positive screening results are corroborated via GC/MS, GC/MS/MS or LC/MS/MS. All samples presumed positive undergo cleaning before confirmation if needed. The comprehensive lab process from Accessioning to Confirmation is audited under both the CAP (College of American Pathologists) Hair classification and follows the accreditation of ISO / IEC 17025 principles.

Advantages of hair drug testing:

  • Extended detection span: Hair tests can perceive drug consumption for up to 90 days, unlike urine tests that have a narrower detection range.
  • Resistant to evasion: Hair drug tests are nearly impossible to deceive, enhancing result accuracy.
  • Chronological data: Reveals a timeline of substance usage over a duration, rather than only recent consumption.

Limitations:

  • Incapable of detecting immediate consumption: It requires roughly 5-7 days for drugs to be traceable in hair.
  • Expense: Hair tests generally incur higher costs compared to alternative drug testing strategies.
  • Result variability: Drug metabolite concentrations in hair can be influenced by hair color and individual growth differences.

Note: Despite being termed "hair follicle tests", the procedure examines the hair strand rather than the hair follicle beneath the scalp.
