For EtG, additional panels, or tests exceeding 10 panels, a sample of 150mg is preferable. We suggest using a jeweler's scale for accuracy. If scalp hair isn't available, an equivalent quantity of body hair is acceptable. "Head hair" refers strictly to scalp hair, whereas "body hair" encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing includes Accessioning, Screening, Extraction, and Confirmation as key steps in drug test analysis.

Accessioning is the preliminary step that logs a sample into the lab's system, ensuring it is securely sealed and dispatched, assigning an LAN (Laboratory Accessioning Number), and completing any remaining data entry absent from an electronic system.

Screening is an initial evaluation for drug abuse; while it is cost-efficient for ruling out drug use in most samples, positive results require confirmation for legal admissibility. Any presumptive positives from Screening necessitate further confirmation.

Presumptive positives from Screening lead to additional sample extraction for further analysis. In Extraction, drugs are isolated from hair at much lower levels compared to other methods like urine or oral fluid, contributing to the complexity of hair drug screening.

Positive results from Screening undergo Confirmation using GC/MS, GC/MS/MS, or LC/MS/MS. Before Confirmation, samples undergo necessary washing. This whole process from Accessioning to Confirmation is thoroughly evaluated following CAP (College of American Pathologists) Hair certification and ISO/IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
