A 1.5-inch segment of around 200 hair strands (akin to the thickness of a #2 pencil) closest to the scalp will yield 100mg, which is the optimal sample for both screening and confirmation purposes. For add-ons like EtG and/or tests exceeding 10 panels, a specimen of 150mg is advisable. It's suggested to weigh the specimen using a jeweler’s scale. In cases where scalp hair is unavailable, a comparable volume of body hair can be gathered. When mentioning head hair, it pertains exclusively to scalp hair. Conversely, body hair is inclusive of all other types (facial, axillary, etc.).

Process Overview

The four principal steps in the laboratory handling of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary integration of a sample into a lab’s system. This process verifies the proper sealing and shipping of the sample, assigns a random LAN (Laboratory Accessioning Number), and fulfills any supplementary data entries absent from an electronic chain of custody system.

Screening performs an initial rapid assessment for drugs of abuse. While Screening is an economical method to exclude drug use in most cases, any positive result must be confirmed for legal admittance. Samples with presumptive positives in Screening require further confirmation.

Should a sample show presumptive positivity during Screening, additional hair is extracted from the initial sample and prepped for Extraction. In this phase, drugs are extracted from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), highlighting why hair drug screening is among the most complex methodologies.

Confirmation of any positive screenings is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples suspected to be positive undergo washing prior to necessary confirmation. The complete laboratory procedure, from Accessioning through Confirmation, undergoes review in accordance with both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.
