Hair analysis is now recognized as an effective instrument for identifying drug and alcohol use. The hair encapsulates a prolonged account of alcohol and other substances by embedding biomarkers within the fibers of the developing hair strand. When taken from near the scalp, hair delivers a detection period of up to nearly three months for alcohol and other substances. Hair collection is straightforward, fairly resistant to tampering, and easy to transport.
A 1.5-inch portion comprising roughly 200 hair strands (around the size of a #2 pencil) closest to the scalp provides 100mg of hair, which is the optimal sample for testing and verification. For EtG, supplemental tests, and/or those exceeding 10 panels, 150mg of the sample is advised. We suggest measuring the sample on a jeweler’s scale. If scalp hair is not present, a comparable quantity of body hair may be used. When mentioning head hair, we mean scalp hair alone. Body hair includes all other hair types (facial, axillary, etc.).
Process Overview
The laboratory processing of a drug test result comprises four principal steps: Accessioning, Screening, Extraction, and Confirmation.
Accessioning consists of the preliminary management of a sample into a laboratory’s system. This involves confirming that the sample was correctly sealed and shipped, issuing a random LAN (Laboratory Accessioning Number), and completing any additional data entry omitted by an electronic chain of custody system.
Screening encompasses an initial quick inspection for substances of abuse. While Screening provides a budget-friendly way to rule out drug usage for most samples, a positive result needs validation for court acceptability. Samples that appear positive in Screening require secondary confirmation.
If a sample tests positive at the Screening phase, more hair is extracted from the original sample and prepared for Extraction. In this step, drugs are extracted from hair at significantly lower concentrations than through other methods (e.g., urine or oral fluid), which makes hair drug screening the most challenging method to perform.
Any positive Screening result is confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive are washed before confirmation as necessary. The complete laboratory process from Accessioning to Confirmation undergoes review both under the CAP (College of American Pathologists) Hair classification and the certification to ISO / IEC 17025 standards.
