A 1.5-inch hair sample comprising roughly 200 strands (approximately the size of a #2 pencil) closest to the scalp yields 100mg, the optimal quantity for analysis. For EtG, additional tests, and/or screenings exceeding 10 panels, 150mg of the sample is advised. It's recommended to measure the sample using a jeweler's scale. If scalp hair isn't available, a comparable amount of body hair might be used. 'Head hair' specifically denotes scalp hair. 'Body hair' encompasses other hair types (facial, axillary, etc.).

Process Overview

The four primary phases in the laboratory handling of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the preliminary integration of a sample into a lab's system. This entails confirming the sample's proper sealing and shipment, assigning a random LAN (Laboratory Accessioning Number), and completing any further data input absent from an electronic chain of custody system.

Screening involves a preliminary assessment for drugs of abuse. Although it serves as an economical means to exclude drug use in most cases, a positive screen must be substantiated for legal admissibility. Samples showing presumptive positivity in Screening require subsequent validation.

Should a sample test presumptively positive during Screening, additional hair is utilized from the original sample and readied for Extraction. During this phase, drugs are extracted from hair at concentrations significantly lower than those in other methods (e.g., urine or oral fluid), making hair drug screening the most complex technique to undertake.

Confirmed positive screenings undergo further examination via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive specimens are cleaned before confirmation as necessary. The comprehensive lab process, from Accessioning to Confirmation, is reviewed under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.
