For EtG, add-ons, and/or tests exceeding 10 panels, a 150mg sample is advisable. Verification of the sample weight with a jeweler’s scale is suggested. If scalp hair is inaccessible, a similar quantity of body hair might be used. Here, head hair is specifically scalp hair. Body hair pertains to all other varieties (facial, axillary, etc.).

Process Overview

There are four key stages in laboratory processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial sample processing into the lab system, ensuring the sample's seal and shipment were intact, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data input missing in the electronic chain of custody.

Screening is a preliminary assessment for abusive substances. While it’s a cost-saving method to exclude drug usage in most samples, for court admissibility, a positive screen must be validated. Samples flagged as presumptively positive during Screening necessitate further confirmation.

Should a sample test potentially positive in Screening, additional hair from the original specimen is extracted for further processing. During Extraction, drugs are isolated from hair at significantly lower concentrations than in alternative methods (e.g., urine or saliva), making hair drug testing particularly intricate.

Confirmation for any positive screening outcomes is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All suspected positive samples undergo a cleansing process before confirmation if necessary. The entire lab procedure from Accessioning to Confirmation is audited under the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
