When obtained near the scalp, hair can present a detection period of up to about 3 months for alcohol and drugs. It is easy to collect hair, challenging to tamper with, and straightforward to transport.

A 1.5-inch segment comprising nearly 200 hair strands (approximately the thickness of a #2 pencil) near the scalp yields 100mg of hair, optimal for screening and confirmation. For EtG, additional tests, and/or tests exceeding 10 panels, 150mg of the specimen is advised. Using a jeweler’s scale to weigh the specimen is advisable. If scalp hair isn’t available, an equivalent volume of body hair can be utilized. When we mention head hair, we refer solely to scalp hair, while body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The core steps in the laboratory treatment of a drug test result comprise Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary processing of a sample into a lab’s system. This incorporates confirming the sample was properly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not facilitated by an electronic chain of custody system.

Screening is an initial fast check for substances of abuse. Despite Screening being a cost-efficient strategy to negate drug consumption in most samples, a positive screen requires validation to be valid in court. Any samples showing presumptive positivity in Screening necessitate secondary verification.

If a sample indicates presumptive positivity in the Screening phase, more hair is taken from the initial sample and readied for Extraction. During this phase, drugs are extracted from hair at significantly reduced concentrations compared to other approaches (e.g., urine or oral fluid), which is why hair drug screening poses the most significant methodological challenges.

Confirmation of any positive screening outcome is executed via GC/MS, GC/MS/MS or LC/MS/MS. All samples with presumptive positivity are cleansed before confirmation as necessary. The full laboratory process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
