A hair sample of 1.5 inches, consisting of roughly 200 strands (approximately the thickness of a #2 pencil) from near the scalp will yield 100mg, the optimal amount for screening and confirmation. For tests involving EtG, add-ons, or more than 10 panels, 150mg is advisable. It's suggested to measure the sample on a jeweler's scale. If scalp hair is unavailable, an equivalent amount of body hair can be used. 'Head hair' specifically refers to scalp hair, while 'body hair' encompasses other types like facial and axillary hair.

Process Overview

The drug testing process in a laboratory consists of four primary steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is first processed into the laboratory's system. This procedure involves checking that the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing additional data entry not handled by an electronic chain of custody system.

Screening is a preliminary, quick check for commonly abused drugs. Although Screening is an economical way to exclude drug use in most cases, a positive result must be confirmed to be valid in legal proceedings. Any sample with a presumptive positive result in Screening requires a secondary confirmation.

Should a sample test presumptively positive in Screening, more hair is extracted from the original sample for the Extraction phase. Here, drugs are extracted from hair at much lower concentrations than other methods (such as urine or oral fluids), making hair drug tests one of the most challenging methodologies to conduct.

Confirmation of any positive Screening result is achieved through GC/MS, GC/MS/MS, or LC/MS/MS techniques. Every presumptive positive sample is washed prior to Confirmation if necessary. The entire process from Accessioning to Confirmation is conducted under both the CAP (College of American Pathologists) Hair accreditation and compliance with ISO / IEC 17025 standards.
