For EtG, additional tests, or panels exceeding 10, a 150mg sample is advised. It is suggested to measure the sample using a jeweler’s scale. If scalp hair is not an option, an equivalent amount of body hair can be taken. Head hair specifies only scalp hair, while body hair includes facial, axillary, and other hair types.

Process Overview

There are four primary stages in lab-based processing of drug test samples: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails integrating a sample into a lab’s database. This consists of confirming proper sealing and shipping, allotting a random LAN (Laboratory Accessioning Number), and completing any missing data not available through an electronic chain of custody.

Screening offers an initial rapid evaluation for potential drug use. Although Screening is cost-effective for excluding drug use in most samples, a positive result must be substantiated for legal admissibility. Any samples flagging as preliminarily positive require additional confirmation.

If a sample tests presumptively positive during Screening, more hair is extracted from the original sample and readied for Extraction. This phase extracts traces of drugs from hair at notably lower levels than other methods (e.g., urine or oral fluid), making hair drug screening particularly challenging to execute.

Confirmation of every positive screen is conducted using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples that test presumptive positive undergo a wash before confirmation if required. The complete lab process from Accessioning to Confirmation is audited under the CAP (College of American Pathologists) Hair criteria and ISO / IEC 17025 standards accreditation.
