When discussing head hair, we refer solely to scalp hair, whereas body hair covers other hair types (such as facial or underarm).

Process Overview

The laboratory analysis of a drug test result encompasses four main phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning starts with processing a sample into a lab’s system, confirming it was correctly sealed and transported, assigning a random Laboratory Accessioning Number (LAN), and finalizing additional data entry if not provided by an electronic chain of custody.

Screening includes a preliminary rapid check for substance abuse. Though Screening is a cost-efficient way to exclude drug use in most samples, a positive result must be confirmed for court acceptance. Any samples indicating potential positivity in Screening necessitate further confirmation.

For samples preliminarily positive in Screening, additional hair is drawn from the original specimen for Extraction. In this measure, drug substances are extracted from hair at much lower concentrations than other methods (e.g., urine or oral fluids), contributing to the complexity of hair drug analysis.

Positive findings in Screening are validated using GC/MS, GC/MS/MS, or LC/MS/MS. All initially positive samples undergo cleaning before confirmation as required. The entire lab process from Accessioning to Confirmation is inspected under the College of American Pathologists (CAP) Hair standards and meets ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
