What it is, How it works

Hair testing has gained recognition as an effective method for identifying drug and alcohol use. Hair captures a comprehensive history of substance use by embedding biomarkers within the fibers of the developing hair strand. When collected near the scalp, hair offers a detection period of up to approximately three months for alcohol and other drugs. Hair is easy to collect, relatively hard to tamper with, and simple to transport.

A 1.5-inch segment comprising about 200 strands of hair (about the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal sample for both screening and confirmation. For EtG add-ons and/or tests exceeding 10 panels, a specimen of 150mg is advised. It is recommended to measure the specimen on a jeweler’s scale. If scalp hair is not available, an equivalent quantity of body hair can be collected. When we mention head hair, we are specifically referring to scalp hair. Body hair pertains to all other hair types (facial, axillary, etc.).

Process Overview

The four primary phases in the laboratory processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial integration of a sample into a laboratory’s system. This step includes ensuring the sample was sealed and transported correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry absent from an electronic chain of custody system.

Screening involves a preliminary rapid assessment for drugs of abuse. While Screening presents a cost-efficient means to exclude drug use in most samples, a positive screening requires confirmation for legal admissibility. Any samples indicating a presumptive positive in Screening necessitate a secondary confirmation.

If a sample reveals a presumptive positive during the Screening phase, additional hair is extracted from the original sample to be prepared for Extraction. During this phase, drugs are isolated from hair at a much lower concentration compared to other methods (such as urine or oral fluid), making hair drug screening the most complex methodology to execute.

Confirmation of any positive screening result is performed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples receive a wash prior to confirmation if necessary. The entire laboratory workflow from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
