In the absence of scalp hair, a comparable quantity of body hair may be used. When mentioning head hair, we reference only scalp hair; body hair encompasses all other hair forms (e.g., facial, axillary).

Process Overview

The laboratory process for drug testing results involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary integration of a sample into a lab's system. The process confirms the sample's secure sealing and transport, assigns a random Laboratory Accessioning Number (LAN), and finalizes any pending data entry absent from an electronic chain of custody system.

Screening comprises an initial swift evaluation for substance misuse. While Screening is economical for excluding drug presence in most instances, a positive result demands confirmation to be court-admissible. Samples that preliminarily test positive during Screening require a follow-up confirmation.

Upon a preliminary positive indication in the Screening phase, additional hair is extracted from the original sample for the Extraction phase. Here, drugs are isolated from hair at significantly lower levels than with other methods (e.g., urine or oral fluids), hence hair drug screen tests are among the most challenging to execute.

Confirmation of any positive results from screening is performed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Presumptive positive samples undergo washing before confirmation if necessary. The whole process from Accessioning to Confirmation is conducted under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
