For EtG tests, additional panels, or tests exceeding 10 panels, 150mg of the specimen is suggested. It is advisable to weigh the specimen using a jeweler's scale. In the absence of scalp hair, a similar quantity of body hair can be collected. When discussing head hair, we specifically mean scalp hair. Body hair encompasses all other types of hair (facial, axillary, etc.).

Procedure Overview

The four key stages in the laboratory assessment of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial integration of a sample into a laboratory's framework. This involves confirming that the sample was appropriately sealed and sent, allocating a unique LAN (Laboratory Accessioning Number), and completing any supplementary data entry not covered by an electronic chain of custody system.

Screening entails a preliminary swift evaluation for drugs of abuse. Although Screening is an economical method to eliminate drug usage from the majority of samples, a positive screen must be confirmed to be acceptable in a judicial context. Any samples indicating a positive result during Screening require a subsequent confirmation.

If a sample shows a positive indication in the Screening phase, additional hair is extracted from the initial specimen and prepared for Extraction. In this phase, drugs are isolated from hair at a significantly lower concentration than in alternative methods (such as urine or oral fluid), which is why hair drug analysis is recognized as the most challenging methodology to execute.

Any positive screening result is corroborated through GC/MS, GC/MS/MS or LC/MS/MS. All samples presumed to be positive are cleansed before confirmation as necessary. The entire laboratory procedure from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.
