What it is, How it works

Hair captures a prolonged record of alcohol and drug use by embedding biomarkers within the structure of the hair strand as it grows. When cut near the scalp, hair can reveal drug and alcohol use for a period of up to three months. It is easy to gather, relatively hard to tamper with, and can be shipped with ease.

A 1.5-inch hair segment, approximately 200 strands (comparable in thickness to a #2 pencil) near the scalp, yields 100mg of hair, an optimal sample for both screening and confirmation purposes. For EtG testing, additional analyses, or tests exceeding 10 panels, a sample weighing 150mg is advised. We suggest using a jeweler’s scale for precise measurement. Should scalp hair be inaccessible, an equivalent volume of body hair may be used. When we mention head hair, we specifically mean scalp hair. Body hair includes all other types (such as facial, axillary, etc.).

Process Overview

The procedure involving the laboratory processing of a drug test result consists of four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of incorporating a sample into a lab's system. This step ensures the sample's integrity by checking if it was sealed and sent correctly, a random LAN (Laboratory Accessioning Number) is assigned, and any extra data entry that an electronic chain of custody system does not supply is completed.

Screening is a preliminary quick assessment for drugs of abuse. Although Screening is an affordable method to exclude drug use for the majority of samples, a positive result in Screening must be validated for it to be permissible in court. Samples that are presumptively positive in Screening indeed need further confirmation.

When a sample is presumptively positive during Screening, additional hair is taken from the initial specimen for the Extraction phase. In this phase, drugs are removed from the hair in smaller concentrations than seen in other methods (such as urine or oral fluid), which is why hair drug testing is considered the most challenging method to conduct.

Confirmation for any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results are cleaned ahead of confirmation as needed. The entire lab procedure from Accessioning to Confirmation is overseen by both the CAP (College of American Pathologists) Hair designation and ISO/IEC 17025 accreditation standards.
