For EtG analyses, additional modules, or tests beyond 10 panels, a 150mg sample is suggested. It is advisable to measure the sample using a jeweler’s scale. In cases where scalp hair is unavailable, an equivalent volume of body hair can be gathered. References to head hair pertain strictly to scalp hair. Body hair encompasses all other types, such as facial and underarm hair.

Process Overview

The laboratory processing of a drug test comprises four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a lab's system. This step verifies proper sealing and shipment of the sample, assigns a random LAN (Laboratory Accessioning Number), and completes any supplementary data entry not covered by an electronic chain of custody system.

Screening consists of an initial rapid assessment for drugs of abuse. Although Screening offers a cost-effective approach to discount drug usage in most samples, a positive result necessitates confirmation for legal admissibility. Presumptively positive samples in Screening require further verification.

If a sample shows a presumptive positive in the Screening phase, additional hair is extracted from the original specimen and prepared for Extraction. During this phase, drugs are isolated from hair at significantly lower concentrations than in alternative methods (e.g., urine or oral fluid), highlighting the complexity of hair drug screening.

Confirmation of any positive Screening result is conducted through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as necessary. The entire lab process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair specification and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
