A 1.5-inch portion consisting of around 200 hair strands (approximately equivalent to a #2 pencil in thickness) close to the scalp provides 100mg of hair, which is the optimal sample amount for analysis and validation. For EtG, additional markers, and/or testing exceeding 10 panels, a 150mg sample is advised. Using a jeweler’s scale to weigh the sample is recommended. If head hair is not accessible, an equivalent quantity of body hair can be collected. When discussing head hair, it specifically pertains to scalp hair. Body hair includes any other types of hair (such as facial or axillary hair).

Process Overview

The laboratory processing of drug test results encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial integration of a sample into the laboratory’s system. This step ensures the sample was properly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and completes any missing data entry from an electronic chain of custody system.

Screening involves a preliminary quick check for drug abuse. While Screening is cost-effective for ruling out drug usage in most samples, a positive result requires confirmation to be acceptable in legal proceedings. Any samples yielding a preliminary positive in Screening require further confirmation.

If a screening result is positive, additional hair is extracted from the primary specimen for Extraction. During this phase, drugs are removed from hair at substantially lower concentrations than other methods (e.g., urine or oral fluid), which is why hair drug screening is considered the most challenging method to execute.

Confirmation of a positive screening result is conducted using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples with a presumptive positive result are cleansed prior to confirmation if necessary. The complete laboratory process from Accessioning to Confirmation is assessed under the CAP (College of American Pathologists) Hair standard and aligned with ISO / IEC 17025 accreditation criteria.
