What it is, How it works

When taken near the scalp, hair can reveal up to a roughly 3-month period of detection for alcohol and other drugs. Hair is straightforward to collect, somewhat tough to tamper with, and simple to transport.

A 1.5-inch section of approximately 200 hair strands (comparable to a #2 pencil's thickness) nearest to the scalp will provide 100mg of hair, which is the ideal sample for testing and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the sample is advised. It is suggested to weigh the sample using a jeweler’s scale. If scalp hair is not available, a comparable amount of body hair can be collected. When discussing head hair, we refer exclusively to scalp hair. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning represents the initial processing of a sample into a laboratory’s system. This step includes confirming that the sample was sealed and transported correctly, assigning a random LAN (Laboratory Accessioning Number), and finalizing any supplementary data entry not provided by an electronic chain of custody system.

Screening is a preliminary rapid check for drugs of abuse. Although Screening is a cost-efficient method to dismiss drug usage on most samples, a positive screening result must be confirmed to stand up in court. Any samples with a presumptive positive in Screening need an additional confirmation.

If Screening indicates a presumptive positive, more hair is retrieved from the initial sample and set for Extraction. In this phase, drugs are drawn from hair at a much lower concentration than other methods (ex. urine or oral fluid), which makes hair drug testing the most challenging methodology to conduct.

Confirmation of any positive screening outcome is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation as necessary. The comprehensive laboratory process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
