A sample consisting of 1.5 inches of approximately 200 hair strands (comparable to the size of a #2 pencil) nearest to the scalp provides 100mg of hair, which is the optimal sample size for both screening and verification. For EtG, additional analyses, and/or tests exceeding 10 panels, 150mg of the sample is advised. It's best to weigh the sample using a jeweler’s scale. If scalp hair is unavailable, a comparable volume of body hair can be used. When discussing head hair, we refer solely to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The lab processing of a drug test involves four crucial stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary handling of a sample in a laboratory setting. This process verifies the proper sealing and shipping of the sample, allocates a unique LAN (Laboratory Accessioning Number), and fulfills any additional data input not completed by an electronic chain of custody system.

Screening consists of an initial rapid assessment for drug abuse. Although Screening is an economical method to exclude drug usage in most samples, a positive screening must be validated to be permissible in court. Any samples that show positive indicators in Screening necessitate a confirmatory test.

In the instance of a presumptive positive during Screening, extra hair is extracted from the original sample and prepared for Extraction. During this phase, drugs are extracted from hair at much lower concentrations compared to other techniques (such as urine or oral fluid), making hair drug screening the most complex to execute.

Confirmation of any positive screening outcome is executed using GC/MS, GC/MS/MS, or LC/MS/MS. Every sample with a presumptive positive result is washed prior to confirmation if necessary. The comprehensive laboratory procedure from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair criteria and the ISO / IEC 17025 accreditation standards.

The joint meeting of the Society of Hair Testing (SoHT) and the Italian Group of Forensic Toxicologists (GTFI) was organized in Verona, June 8–10, 2022, and hosted by the Legal Medicine team of the Verona University Hospital, directed by Franco Tagliaro together with Federica Bortolotti and Rossella Gottardo.

It served as a prominent platform for knowledge exchange and collaboration, facilitating groundbreaking advancements in hair testing and analysis. In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
