For EtG, additional tests, and tests exceeding 10 panels, 150mg of hair is suggested. It is advisable to measure the sample on a jeweler’s scale. If scalp hair cannot be accessed, a comparable quantity of body hair is acceptable. References to head hair specifically mean scalp hair. Body hair encompasses other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result consists of four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage of introducing a sample into the laboratory framework. This includes ensuring the sample is properly sealed and transported, assigning a unique LAN (Laboratory Accessioning Number), and completing any extra data input not provided by an electronic chain of custody system.

Screening performs an initial examination for drug presence. Though it is a cost-efficient method to exclude drug use in most specimens, any positive results from Screening require verification to be court-admissible. Samples that are presumptively positive in Screening need further verification.

For samples deemed presumptively positive in Screening, additional hair is extracted from the preliminary sample for the Extraction stage. Here, drugs are extracted from hair at much lower concentrations than in other methods (such as urine or oral fluid), making hair drug testing the most challenging process to conduct.

Confirmation of any positive findings from Screening is done using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples that are presumptively positive are cleansed before confirmation as necessary. The complete laboratory workflow from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.
