What it is, How it works

Hair testing has gained recognition as a robust method for detecting drug and alcohol consumption. Hair stores a long-term record of drug and alcohol intake by capturing biomarkers in the fibers of the growing hair strand. When collected near the scalp, hair can offer up to a 3-month detection period for alcohol and drugs. Hair is straightforward to collect, challenging to tamper with, and convenient to transport.

A 1.5-inch sample comprising approximately 200 hairs (around the diameter of a #2 pencil) nearest to the scalp provides 100mg of hair, which is considered the ideal sample size for screening and confirmation. For EtG, add-ons, and/or tests over 10 panels, a 150mg specimen is recommended. Measuring the sample on a jeweler’s scale is advisable. If scalp hair is unavailable, an equivalent amount of body hair may be used. "Head hair" refers exclusively to scalp hair, while "body hair" encompasses all other types (facial, axillary, etc.).

Process Overview

The four primary steps in the lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial intake of a sample into the lab’s system. It includes confirming proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and finalizing any additional data input not supplied by an electronic chain of custody system.

Screening provides a preliminary, efficient check for drug presence. While it's a cost-effective means to exclude drug use in most samples, a positive screen must be confirmed to be legally valid. Samples testing presumptively positive in Screening require a follow-up confirmation.

If a sample tests presumptively positive during Screening, additional hair is extracted from the initial sample and prepped for Extraction. During this phase, drugs are extracted from hair at much lower concentrations than in other methods (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to execute.

Confirmation of any positive screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation if needed. The entire lab process from Accessioning through Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO/IEC 17025 standards.

The impact of hair treatments on the oxidation/decomposition of drugs in hair was also explored, as the long-term fate of drugs incorporated in the keratin matrix. Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
