When testing for EtG, add-ons, and/or panels over 10, a 150mg sample is advisable. Using a jeweler’s scale for weighing the sample is suggested. In the absence of scalp hair, an equivalent quantity of body hair may be retrieved. When referencing head hair, we specifically mean scalp hair. In contrast, body hair refers to all alternative types of hair (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary incorporation of a sample into a laboratory’s system. This process ensures the sample was sealed and dispatched correctly, assigns a random LAN (Laboratory Accessioning Number), and completes any additional data input not covered by an electronic chain of custody system.

Screening constitutes an initial rapid assessment for drugs of abuse. While Screening is a cost-efficient strategy to dismiss drug usage for most samples, a positive screen must undergo confirmation to hold up in court. Any samples presumed positive during Screening necessitate a secondary confirmation.

For samples that appear positive during the Screening phase, more hair is extracted from the original sample and readied for Extraction. Here, drugs are extracted from the hair at significantly lower concentrations than other methods (such as urine or oral fluid), making hair drug testing the most challenging methodology to execute.

GC/MS, GC/MS/MS, or LC/MS/MS methods confirm any positive screening outcomes. All presumptive positive samples undergo a washing process before confirmation if necessary. The complete laboratory procedure, from Accessioning to Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair designation and the ISO/IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
