For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the sample is suggested. We advise weighing the sample on a jeweler’s scale. If scalp hair is lacking, a similar quantity of body hair can be used. By head hair, we mean only scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The four primary steps involved in processing a drug test result in a laboratory are Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial input of a sample into the lab’s system. This step ensures that the sample was correctly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and completes any missing data typically entered from an electronic chain of custody system.

Screening entails an initial rapid assessment for drugs of abuse. Screening is a cost-efficient method to exclude drug usage from most samples; however, a positive screen must be confirmed for legal admissibility. Presumptive positive samples in Screening require a subsequent confirmation.

If a sample shows a presumptive positive during Screening, more hair is prepared from the initial batch for Extraction. During Extraction, drugs are separated from hair at a notably lower concentration compared to other testing methods (e.g., urine or oral fluid), making hair drug screening the most challenging method to perform.

Confirmation for any positive screen result is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all presumptive positive samples are cleaned as necessary. The overall laboratory process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair criteria and accreditation to ISO / IEC 17025 standards.
