For EtG, additional components, or tests exceeding 10 panels, 150mg of the sample is advisable. It is suggested to weigh the sample using a jeweler’s scale. If scalp hair is inaccessible, a comparable quantity of body hair can be collected. Head hair specifically refers to scalp hair, while body hair includes other types (e.g., facial, axillary).

Process Overview

The process in drug testing at the laboratory mainly includes four steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first step of integrating a sample into a lab’s framework. This entails confirming the sample was correctly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and completing extra data entry that an electronic chain of custody system might not offer.

Screening serves as an initial rapid check for drug abuse. Although Screening is a cost-efficient method to exclude drug use in most samples, a positive screen must be validated for legal admissibility. Samples that show presumptive positivity in Screening require further confirmation.

In cases where a sample tests presumptively positive during Screening, additional hair is obtained from the original specimen for Extraction. During Extraction, drugs are withdrawn from hair at a much lesser concentration than other methods (such as urine or oral fluid), making hair drug screening quite challenging to execute.

A positive Screening result undergoes Confirmation through GC/MS, GC/MS/MS, or LC/MS/MS. Samples that are presumptively positive are cleansed before confirmation if necessary. The full process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
