Obtaining a 1.5-inch section around 200 hair strands (comparable to a #2 pencil) nearest to the scalp offers 100mg of hair, an optimal amount for analysis and confirmation tests. For EtG, supplementary tests, or testing that exceeds 10 panels, a recommendation of 150mg is given. A jeweler’s scale can be utilized to weigh the specimen accurately. If the scalp is not an option, an equivalent sample of body hair can be utilized. The term head hair strictly refers to hair from the scalp, while body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The processing of a lab-based drug screening result involves four major phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning incorporates the initial introduction of a specimen into a lab’s system, ensuring proper seal and shipment, allotting a unique LAN (Laboratory Accessioning Number), and completing any absent data that isn't automatically captured by an electronic chain of custody system.

Screening provides a preliminary inspection for substance abuse. Although Screening represents an economical method for excluding drug use in the majority of specimens, any positive indications require confirmation for judicial acceptability. Specimens with a tentative positive result in Screening necessitate subsequent verification.

Once a specimen indicates preliminary positive results during Screening, additional hair is extracted from the collection and prepared for Extraction. This phase involves retrieving drugs from hair at considerably lower concentrations compared to other techniques (e.g., urine or oral fluid), which makes hair drug screening inherently challenging to execute.

Confirmation of any presumptive positive screening involves using GC/MS, GC/MS/MS, or LC/MS/MS technology. Presumptive positive samples undergo cleansing as required before Confirmation. The comprehensive lab procedure from Accessioning to Confirmation adheres to the CAP (College of American Pathologists) Hair criteria and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
