For EtG, additional substances, and/or tests exceeding 10 panels, 150mg of specimen is advisable. Using a jeweler’s scale to weigh the specimen is suggested. If scalp hair isn't available, a comparable quantity of body hair may be collected. When mentioning head hair, we specifically refer to scalp hair. Body hair encompasses all other hair varieties (facial, axillary, etc.).

Process Overview

The primary stages in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the sample's initial entry into a lab's system. This process ensures the sample was sealed and transported correctly, a random LAN (Laboratory Accessioning Number) is assigned, and any extra data entry not available through an electronic chain of custody system is completed.

Screening is a preliminary evaluation to detect drug use. Although Screening is an economical way to eliminate drug use in most samples, a positive result requires confirmation to be valid in court. Samples that are tentatively positive in Screening necessitate a secondary confirmation.

If the Screening phase indicates a presumptive positive, more hair is drawn from the original specimen and readied for Extraction. Here, drugs are extracted from the hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), which makes hair drug screening the most challenging technique.

Verification of any positive screening outcome is conducted through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation if necessary. The entire lab process from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair standard and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
