What it is, How it works

Hair analysis has gained recognition as an effective method for identifying drug and alcohol consumption. Hair retains a lasting record of alcohol and drugs by embedding biomarkers within the hair's growing strands. When gathered near the scalp, hair can reveal a detection period of roughly three months for alcohol and drugs. It is straightforward to collect, relatively resistant to being tampered with, and simple to transport.

A 1.5-inch portion consisting of about 200 hair strands (comparable to the thickness of a #2 pencil) that is closest to the scalp will yield 100mg of hair, which is the optimal sample size for testing. For EtG, additional tests, and/or assessments exceeding 10 panels, 150mg of the sample is suggested. It is advised to measure the sample using a jeweler’s scale. If scalp hair is not available, a similar quantity of body hair may be gathered. The term head hair specifically denotes scalp hair only. Body hair pertains to all other types (facial, axillary, etc.).

Process Overview

The four principal stages in the lab analysis of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary incorporation of a sample into a lab’s system. This process entails confirming the sample was properly sealed and dispatched, allocating a random LAN (Laboratory Accessioning Number), and completing any extra data entry not supplied by an electronic custody system.

Screening constitutes a preliminary assessment for drugs of misuse. While Screening is a cost-efficient means to eliminate drug use in most samples, a positive screen requires confirmation for court admissibility. Any samples that test presumptively positive in Screening demand a secondary confirmation.

During this step, drugs are extracted from hair at a much lower concentration compared to other techniques (e.g., urine or oral fluid), which is why hair drug testing is among the most complex methodologies.

Confirmation of any positive Screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo a wash prior to confirmation when necessary. The entire laboratory sequence from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
