A 1.5-inch piece, comprising about 200 hair strands (similar in size to a #2 pencil) closest to the scalp, yields 100mg of hair, which is the preferred sample size for assessment and validation. For EtG, additional tests, and/or more extensive panels beyond ten, 150mg of the sample is advisable. We suggest using a jeweler’s scale to measure the sample weight. If scalp hair is absent, an equivalent quantity of body hair is acceptable. When mentioning head hair, we specifically mean scalp hair. Body hair refers to hair from all other areas (facial, axillary, etc.).

Process Overview

Accessioning encompasses the initial integration of a sample into the lab’s database. This involves checking that the sample was securely sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data input not facilitated by an electronic chain of custody.

Screening includes an initial rapid assessment for drug abuse. While Screening is a cost-efficient approach to eliminate drug usage from most samples, a positive result must be verified to be acceptable in legal proceedings. Samples with a presumptive positive outcome in Screening require further confirmation.

If a sample shows a presumptive positive at the Screening phase, additional hair is taken from the initial sample and readied for Extraction. During this phase, drugs are extracted from hair at significantly lower concentrations compared to other techniques (e.g. urine or oral fluid), which is why hair drug screening is notably complex.

Confirming any positive outcome in Screening is carried out using GC/MS, GC/MS/MS, or LC/MS/MS technology. All samples with presumptive positive results are cleansed before confirmation if necessary. The comprehensive laboratory proceedings from Accessioning to Confirmation are assessed according to both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
