What it is, How it works

Hair testing has emerged as an effective approach for identifying drug and alcohol usage. Hair offers a detailed record of alcohol and drug consumption by holding biomarkers within the fibers of the hair as it grows. When taken from near the scalp, hair can reveal a detection period of up to around three months for alcohol and various drugs. Hair is straightforward to gather, fairly challenging to tamper with, and simple to transport.

A 1.5-inch segment of approximately 200 hair strands close to the scalp will yield 100mg of hair, which is the optimal sample for analysis and confirmation. For EtG, additional tests, and/or analysis surpassing 10 panels, it is suggested to collect 150mg of the sample. We suggest using a jeweler’s scale to measure the sample. If scalp hair cannot be collected, an equivalent quantity of body hair may be used. When mentioning head hair, it refers exclusively to scalp hair. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The main stages in the laboratory handling of a drug test result encompass Accessioning, Screening, Extraction, and Confirmation.

Accessioning comprises the initial handling of a sample within a lab's system. This encompasses ensuring the sample was correctly sealed and delivered, allocating a random LAN (Laboratory Accessioning Number), and finalizing any remaining data entry not handled by an electronic chain of custody system.

Screening involves a preliminary evaluation for drugs of abuse. Although Screening is an economical means to exclude drug usage in most samples, a positive result necessitates confirmation to be valid for legal proceedings. Any samples that show presumptive positives in Screening require further confirmation.

If a sample is potentially positive during the Screening phase, additional hair is extracted from the original sample and readied for Extraction. At this phase, drugs are isolated from hair at a substantially lower concentration than with other methods (ex. urine or oral fluid), making hair drug screening the most challenging technique to execute.

Confirmation of any positive Screening outcome is carried out through GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive are cleaned prior to confirmation as needed. The entire lab process from Accessioning to Confirmation adheres to both the CAP (College of American Pathologists) Hair designation and ISO/IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
