A sample of 1.5 inches consisting of around 200 strands (approximately the size of a #2 pencil) nearest to the scalp will yield 100mg, the preferred amount for analysis and verification. For EtG, extra tests, or those involving more than 10 panels, 150mg of the sample is advisable. Using a jeweler’s scale to weigh the sample is suggested. If scalp hair is not available, a similar amount of body hair can be used. "Head hair" refers exclusively to scalp hair. "Body hair" includes all other types (facial, axillary, etc.).

Process Overview

The laboratory procedure for processing a drug test result comprises four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a lab’s system. This step verifies proper sealing and shipping of the sample, assigns a random LAN (Laboratory Accessioning Number), and handles any additional data input not managed by an electronic custody chain.

Screening provides a preliminary evaluation for potential drug abuse. Although Screening is a budget-friendly method to rule out drug use for most samples, a positive result must be confirmed for legal admissibility. Samples flagged as presumptively positive in Screening need a follow-up confirmation.

If Screening suggests a positive result, additional hair is extracted from the primary sample and prepped for Extraction. During this phase, drugs are removed from hair at much lower concentrations compared to methods like urine or oral fluid, highlighting the complexity of hair drug screening.

Verification of any positive screening outcome is done through GC/MS, GC/MS/MS, or LC/MS/MS. Samples indicated as presumptively positive undergo a washing process prior to confirmation as necessary. The full lab procedure from Accessioning to Confirmation adheres to both CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
