For EtG, add-ons, and/or screenings beyond 10 panels, a sample of 150mg is advised. It's recommended to use a jeweler’s scale for measuring the specimen. Should scalp hair be inaccessible, a corresponding volume of body hair may be taken. By head hair, we mean only scalp hair. Body hair pertains to all other hair varieties (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a lab’s system. This step involves confirming that the sample was correctly sealed and dispatched, assigning a unique LAN (Laboratory Accessioning Number), and finalizing any extra data input absent from an electronic chain of custody system.

Screening constitutes an initial rapid check for drug abuse. While Screening is an economical way to eliminate drug usage in most samples, a positive screen necessitates confirmation for legal admissibility. Any samples testing positive in Screening require a subsequent confirmation.

If a sample is deemed positive at the Screening phase, additional hair is taken from the original sample for Extraction. During this phase, drugs are extracted from hair at considerably lower concentrations than in other methods (e.g., urine or oral fluid), making hair drug screening the most intricate methodology to execute.

Positive screening results undergo Confirmation via GC/MS, GC/MS/MS, or LC/MS/MS. All positive samples are cleansed before confirmation as needed. The complete lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair classification and the ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
