For EtG, additional tests, and/or panels over 10, a 150mg sample is advisable. We endorse using a jeweler's scale for measurement. If scalp hair is inaccessible, an equivalent quantity of body hair is acceptable. In reference to head hair, we specify scalp hair. Body hair encompasses other types such as facial and axillary.

Process Overview

The crucial phases in lab processing for drug testing include Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial incorporation of a sample into a laboratory's system. This step ensures the sample’s proper sealing and shipping, assigns a unique LAN (Laboratory Accessioning Number), and completes any extra data input absent in an electronic chain of custody setup.

Screening consists of a preliminary check for drug substances. Although Screening is an economical method to rule out drug usage in most samples, a positive screen must be verified for legal acceptance. Samples with presumed positives in Screening require further confirmation.

When a sample is presumably positive during Screening, additional hair is extracted from the original sample for Extraction. During this phase, drugs are extracted at much lower concentrations compared to other methods (such as urine or oral fluid), which makes hair drug screening particularly challenging.

Positive screening results are verified using GC/MS, GC/MS/MS, or LC/MS/MS. Before Confirmation, all presumptive positive samples are washed if necessary. The entire process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standard accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
