What it is, How it works

When taken from near the scalp, hair can offer up to roughly a 3-month detection period for alcohol and drugs. Hair is easy to collect, relatively hard to tamper with, and convenient to ship.

An ideal sample for testing and validation involves a 1.5-inch section, or about 200 hair strands (comparable to a #2 pencil in size), gathered closest to the scalp, yielding 100mg of hair. For EtG, additional testing, or tests beyond 10 panels, 150mg of the sample is advised. It is recommended to weigh the sample using a jeweler’s scale. If scalp hair isn't available, a comparable quantity of body hair may be used. "Head hair" refers strictly to scalp hair. "Body hair" includes all other types (facial, axillary, etc.).

Process Overview

The main stages in laboratory processing of drug test outcomes include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the primary processing of a sample into a lab’s system. This process confirms the sample was properly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and completes any other data entry that an electronic chain of custody system doesn't cover.

Screening conducts an initial quick check for substance abuse. Screening is cost-effective for dismissing drug use in most samples; however, a positive screen must be validated for court acceptance. Any samples tentatively positive during Screening must undergo secondary validation.

If a sample is tentatively positive in the Screening phase, more hair is taken from the original sample and processed for Extraction. Here, drug extraction from hair occurs at lower concentrations than other methods (such as urine or saliva), making hair drug screening a challenging methodology.

Confirmation of any positive screening results is done via GC/MS, GC/MS/MS or LC/MS/MS. Before confirmation, all presumptive positive samples are washed as necessary. The full laboratory procedure from Accessioning to Confirmation is overseen under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
