For EtG, additional tests, and those exceeding 10 panels, 150mg of the sample is advisable. It is suggested to weigh the sample using a jeweler’s scale. If scalp hair is not present, an equivalent portion of body hair can be utilized. The term head hair pertains solely to scalp hair, while body hair encompasses all other types (such as facial, axillary, etc.).

Process Overview

The laboratory processing of hair drug tests involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

The Accessioning step captures the initial handling of a sample within a lab's system. This stage ensures that the sample was securely sealed and correctly transported, a random LAN (Laboratory Accessioning Number) is allocated, and any additional data input not covered by an electronic chain of custody is completed.

Screening entails a preliminary swift test for illicit drugs. Although Screening is economical for ruling out drug involvement in most samples, a positive screen must be verified for it to be valid in legal contexts. Samples with a presumptive positive result in Screening necessitate a further confirmation.

When a sample shows a presumptive positive at the Screening phase, additional hair from the original sample is prepared for Extraction. During this phase, drug substances are extracted from the hair at much lower levels than alternative methods (e.g., urine or oral fluids), which is why hair drug testing is among the most challenging processes.

Confirmation of any positive screen results is accomplished using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples showing a presumptive positive are cleansed before confirmation if needed. The full lab process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation.
