What it is, How it works

Hair analysis is now recognized as an effective measure for identifying drug and alcohol consumption. Hair encapsulates long-term evidence of alcohol and other substances by embedding biomarkers within the hair strand fibers as they grow. When taken from near the scalp, hair can offer up to a 3-month period for detecting alcohol and other substances. Collecting hair is straightforward, tampering is relatively challenging, and transportation is uncomplicated.

A 1.5-inch section of roughly 200 hair strands (roughly equivalent to a #2 pencil), cut closest to the scalp, yields 100mg of hair, the prime amount for both screening and confirmation processes. For EtG, additional tests, or screenings exceeding 10 panels, a specimen of 150mg is advisable. Using a jeweler's scale to weigh the specimen is suggested. If scalp hair is not an option, an equivalent amount of body hair can be utilized. Mention of head hair pertains exclusively to scalp hair. Body hair includes any other types of hair (facial, axillary, etc.).

Process Overview

The drug test laboratory processing encounters four principal phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary phase where a sample is recorded into a laboratory's system. This consists of confirming the proper sealing and shipping of the sample, assigning a unique LAN (Laboratory Accessioning Number), and completing any extra data entry not supplied by an electronic chain of custody system.

Screening is an initial rapid assessment for drug abuse. Although Screening is a budget-friendly means to eliminate drug usage in most cases, a positive screen must be validated to hold up in legal proceedings. Samples that are tentatively positive during Screening necessitate a subsequent confirmation.

If Screening yields a tentative positive, further hair is taken from the original specimen for Extraction. This phase involves withdrawing drugs from hair at notably lower concentrations than in other methods (e.g., urine or oral fluid), marking hair drug screening as the most challenging method to execute.

Confirmation of any positive screening outcome is carried out through GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all samples with tentative positive results are washed as required. The complete laboratory procedure from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and the standards of ISO / IEC 17025 accreditation.
