For EtG, extended tests, or those beyond 10 panels, a sample size of 150mg is advised. Using a jeweler's scale to weigh the sample is recommended. If scalp hair cannot be collected, a similar quantity of body hair is acceptable. When discussing head hair, we specifically mean scalp hair, whereas body hair encompasses other hair types (facial, axillary, etc.).

Process Overview

The laboratory procedure for analyzing a drug test result comprises four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning denotes the initial sample intake process within a laboratory setting, ensuring the proper sealing and shipment of the sample, the assignment of a unique LAN (Laboratory Accessioning Number), and the completion of any necessary data entry absent from an electronic chain of custody.

Screening serves as a preliminary inspection for drug traces. This stage is both cost-efficient and effective for dismissing drug presence in most samples, although any positive result in Screening requires subsequent confirmation for legal acceptability. Samples found to be tentative positives in Screening necessitate further confirmation.

For samples flagged as potentially positive in Screening, additional hair is extracted from the primary sampling and prepped for the Extraction phase. During this stage, drug compounds are extracted at significantly lower concentrations than other testing methods (e.g., urine or oral fluid), making hair drug testing notably challenging.

Confirmation of a positive screening involves GC/MS, GC/MS/MS, or LC/MS/MS techniques. Any suspect samples undergo a wash before proceeding to confirmation if necessary. The complete lab protocol from Accessioning through to Confirmation undergoes a review adhering to both CAP (College of American Pathologists) Hair classification and the ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
