If gathered near the scalp, it can reveal substance usage up to a 3-month duration. Hair samples are easy to collect, not easily tampered with, and can be transported without difficulty.

A sample of 1.5 inches consisting of roughly 200 hair strands (comparable to a #2 pencil) nearest the scalp yields 100mg, which is the optimal quantity for testing and validation. For EtG, additional testing, and/or panels exceeding 10, a collection of 150mg is suggested. Using a jeweler's scale for weighing the specimen is recommended. In the absence of scalp hair, an equivalent volume of body hair can be used. "Head hair" refers exclusively to scalp hair, while "body hair" includes all other types (including facial, axillary, etc.).

Process Overview

There are four critical phases in lab processing of drug test outcomes: Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the preliminary integration of a sample into a laboratory's framework. This phase confirms the sample's proper sealing and shipping, assigns it a random LAN (Laboratory Accessioning Number), and concludes any outstanding data logging not accomplished by an electronic chain of custody system.

Screening serves as an initial rapid assessment for substance misuse. While it is a cost-conscious strategy for excluding drug presence in the majority of cases, any positive screening necessitates validation for legal acceptance. Any samples that present potential positive results in Screening require further confirmation.

If Screening suggests a presumptive positive result, additional hair is extracted from the original sample and readied for Extraction. At this step, substances are isolated from hair at far more dilute concentrations compared to alternative techniques (such as urine or oral fluids), underscoring the complexity of hair drug screening procedures.

GC/MS, GC/MS/MS, or LC/MS/MS methods confirm any positive screening findings. Any samples flagged as presumptive positives are cleansed ahead of confirmation, if necessary. The complete laboratory sequence, from Accessioning to Confirmation, adheres to the CAP (College of American Pathologists) Hair accreditation as well as ISO/IEC 17025 certification standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
