A sample measuring 1.5 inches, or about 200 hair strands (roughly comparable to the thickness of a #2 pencil) closest to the scalp, yields 100mg, which is optimal for both screening and confirmation. For EtG, extras, and/or analyses beyond 10 panels, 150mg of the sample is advisable. We suggest using a jeweler’s scale to measure the sample. If scalp hair cannot be provided, a similar quantity of body hair can be used. References to head hair specifically denote scalp hair. Body hair includes all other variations (facial, axillary, etc.).

Steps to the Process

The primary phases in laboratory processing a drug result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the introductory phase of sample integration into a lab’s framework. This step encompasses verifying the sample's proper sealing and shipping, assigning a unique LAN (Laboratory Accessioning Number), and finalizing any outstanding data inputs absent in an electronic custody chain system.

Screening involves an initial rapid evaluation for substances of abuse. Screening is an economical method to negate drug consumption for most samples; however, any positive screen needs verification for legal acceptability. Any samples preliminarily positive in Screening necessitate an additional confirmation.

If a sample tests as presumptively positive during Screening, further hair is retrieved from the originating specimen and readied for Extraction. During this phase, substances are extracted from hair at a notably reduced concentration than other methods (e.g., urine or oral fluid), which positions hair drug screening as one of the challenging methodologies.

Validation of any positive screen is completed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples undergo cleansing prior to confirmation as applicable. The complete lab procedure from Accessioning to Confirmation gets assessed under the CAP (College of American Pathologists) Hair designation and accredited against ISO / IEC 17025 standards.
